The ECHS scheme does not only cover the retired ex-serviceman — it extends comprehensive healthcare benefits to the entire enrolled family. For Gorkha veterans living in Nepal, ensuring that your wife, children, and dependent parents are correctly enrolled on your ECHS Smart Card is critical. This guide explains who qualifies as a dependent, how to register them, and exactly what medical coverage they receive.

Who Qualifies as a Dependent Under the ECHS Scheme?

ECHS recognises the following as eligible dependents: the lawfully married spouse of the ex-serviceman, unmarried children up to age 25 (or any age if permanently disabled and fully dependent), and parents wholly dependent on the ESM. Each dependent must be formally enrolled on the ECHS Smart Card to access benefits.

Detailed eligibility by category:

  • Spouse (wife): The legally married wife of the ESM is automatically eligible for enrollment. In cases of second marriages or widowed ESM who remarry, the current spouse is the entitled dependent. The spouse’s name must be on the ECHS Smart Card.
  • Unmarried daughters and sons: Children up to the age of 25 years who are unmarried and dependent on the ESM are eligible. Once a child marries or turns 25 (and is no longer in education or unemployed), they must be removed from the card.
  • Permanently disabled children: Children of any age who are permanently and wholly dependent on the ESM due to physical or mental disability remain eligible. Medical certification of the disability is required.
  • Dependent parents: Father and/or mother of the ESM who are wholly dependent on the ESM — financially and residentially — are eligible. This requires a dependency certificate (affidavit) and proof that they are not covered under any other government health scheme.

All dependents must be enrolled at the time of ECHS card issue or updated through a card amendment. Dependents not listed on the card cannot be covered cashlessly.

How Do You Register Dependents on the ECHS Smart Card?

Dependents are registered at the time of initial ECHS card application, or through a card amendment application at any time during the card’s validity. The ESM submits required documents for each dependent to the Regimental Records Office, Station HQ, Zila Sainik Daftar, or the Embassy of India Defence Wing in Kathmandu.

Documents required for dependent registration:

  • Spouse: Marriage certificate, spouse’s passport-size photograph, spouse’s photo ID (Nepali citizenship or passport).
  • Children: Birth certificate, photograph, school enrollment certificate (if applicable), dependency declaration by ESM.
  • Dependent parents: Dependency certificate/affidavit (notarised or stamped by local authority), parents’ photo ID, parents’ photographs, declaration that they are not covered under any other central government health scheme.
  • Permanently disabled child: Medical board certificate confirming permanent disability, birth certificate, photograph.

After submitting documents, the updated ECHS Smart Card is produced and dispatched within 30–90 days. Until the updated card arrives, the ESM can carry the acknowledgement of the amendment application to explain the pending update at the polyclinic.

What Healthcare Coverage Does the ESM’s Spouse Receive?

The enrolled wife of an ESM receives the full range of ECHS benefits — OPD consultations, inpatient care, surgical procedures, diagnostic investigations, medicines, specialist referrals, and maternity services. Her healthcare entitlement is equivalent to the ESM’s own entitlement, accessed through the same ECHS card.

Specific coverage for the spouse includes:

  • General OPD: Consultations for all medical conditions at the polyclinic.
  • Specialist care: Gynecology, obstetrics, general medicine, orthopedics, ENT, ophthalmology — all through referral to empanelled hospitals.
  • Maternity and obstetric care: Antenatal care, normal and caesarean delivery, postnatal care — covered at empanelled hospitals. This is a particularly important benefit for younger dependents.
  • Gynecological procedures: Hysterectomy, fibroid removal, ovarian cyst surgery, and other gynecological surgical procedures are covered.
  • Investigations: All investigations ordered for her care — blood tests, ultrasound, pap smear, mammogram — covered within ECHS entitlements.
  • Medicines: All formulary medicines relevant to her conditions.

The spouse accesses services by presenting the ECHS Smart Card (showing her enrollment) and, for non-emergency visits, the polyclinic referral letter.

What Pediatric Coverage Do Enrolled Children Receive?

Enrolled children receive the full range of pediatric healthcare under ECHS — consultations, investigations, medicines, hospitalisation, and specialist referrals for childhood conditions. Coverage includes treatment for common childhood illnesses, surgical conditions, vaccinations listed in ECHS guidelines, and management of chronic childhood diseases.

Pediatric conditions covered:

  • Acute febrile illnesses — fever, malaria (in endemic areas), typhoid, respiratory infections
  • Gastrointestinal conditions — diarrhea, vomiting, abdominal pain, constipation
  • Respiratory conditions — asthma, pneumonia, bronchitis
  • Ear, nose, and throat — otitis media (ear infections), tonsillitis, sinusitis
  • Surgical conditions — hernia, appendicitis, intussusception, undescended testis
  • Developmental and nutritional assessment
  • Chronic conditions — diabetes (Type 1 in children), epilepsy, congenital heart disease (referral to higher centre)

How Do You Add a Newborn to the ECHS Card?

Until the updated card arrives, the birth certificate can serve as documentation for polyclinic registration.

Steps to add a newborn:

  1. Obtain the birth certificate (from the ward office or hospital).
  2. Complete the ECHS card amendment form (available from Zila Sainik Daftar or echs.gov.in).
  3. Attach the birth certificate, a photograph of the newborn (when available), and a letter from the ESM requesting enrollment.
  4. Submit to the Zila Sainik Daftar, Records Office, or Embassy Defence Wing.
  5. The updated card with the newborn listed is dispatched within 30–90 days.
  6. In the interim, carry the birth certificate and hospital birth record when accessing polyclinic or hospital services for the child.

What Happens When a Dependent Is No Longer Eligible?

When a dependent ceases to be eligible — child turns 25 or marries, parent passes away, or a marriage ends — the ESM must inform the Records Office and have the dependent removed from the ECHS card. Failure to remove ineligible dependents is a breach of ECHS rules and can result in card suspension.

Situations requiring dependent removal:

  • Child reaches 25: Must be removed unless permanently disabled and wholly dependent.
  • Child marries: Must be removed from ESM’s card on marriage.
  • Child becomes employed: When a child becomes financially independent, remove from card.
  • Death of dependent: Report the death and submit death certificate for card amendment.
  • Separation / divorce: ECHS guidelines specify the position in such cases — consult the Records Office or Embassy Defence Wing.

The process involves submitting a card amendment request with the relevant supporting document (marriage certificate, death certificate, etc.) to the Records Office.

Can Dependent Parents Access Maternity or Pediatric Services?

No. Maternity services apply to the ESM’s enrolled spouse only. Pediatric services apply to enrolled children. Dependent parents receive medical, surgical, and specialist coverage appropriate to adult/elderly healthcare — general medicine, orthopedics, cardiac care, ophthalmology, and other adult medical services.

Dependent parents commonly need care for:

  • Hypertension and diabetes management
  • Cardiac conditions
  • Orthopedic conditions (hip and knee OA, osteoporosis, fractures)
  • Cataract surgery and eye care
  • Respiratory conditions (COPD, asthma)
  • Neurological conditions (stroke management, dementia care)

All these are covered under ECHS for enrolled dependent parents, following the same polyclinic-referral-empanelled hospital pathway.
